Please mind the Questions 3 & 7 of the visa application form: Questions 3a & 3b require the applicant's permanent address in Germany and home phone number, while in Question 7 you have to provide the full name, address and the phone number of a contact in Ghana.
The minor applicants (under 18) have to provide a Parental Permission and passport copies (of both parents).

To apply for a travel visa to Ghana requires the following documents:
must be valid for at least six (6) months from the date of departure. All passports less than six (6) months from date of travel are unacceptable. Passports must bear signatures of bearers.
Biometric photo (600x600 pixels) against a light (white or light blue) background.
Yellow Fever Vaccination.
Copy of International Certificate of Vaccination for Yellow Fever.
Copy of a recent bank statement showing proof of sufficient funds.
Letter from present employer or educational institution confirming status. If self-employed, particulars of business registration
Letter of invitation from person or institution in Ghana.
Applicants under 18 years, require an authorisation letter from both parents and photocopy of parent's passports.
Photocopy of the bio-data page of applicant’s passport.
Passport copy of host.
If residing with a private person in Ghana a copy of passport of the host in Ghana if Ghanaian; copy of passport and proof of residence in Ghana for non-Ghanaian hosts mus be provided.
